Placing a loved one or moving into an aged care home can be a daunting experience. The following step by step process will assist you with applying for respite and permanent accommodation at Hill View House.
Step 1. You will require an Aged Care Client Record (‘ACCR’). This can be obtained from the Aged Care Assessment Team (‘ACAT’). ACAT can be contacted directly to book an assessment. Once ACAT have undertaken the assessment a copy of the ACCR will be provided to the resident and/or their representative. A copy of the ACCR will need to be provided to Hill View House.
Step 2. Please complete a Hill View House application form.
Step 3. You may have an Enduring Power of Attorney (‘EPOA’) in place. A certified copy of the Enduring Power of Attorney must also be provided.
Step 4. Once all required documentation has been received the Hill View House admissions team will contact you to assist you with the rest of the admission process.
