Admission Requirements
Aged Care Assessment
Before a person is approved as a Care Recipient (Permanent or Respite) they must be assessed by the Aged Care Assessment Team (ACAT).
Aged Care Assessment Team is located at: Gold Coast Hospital Phone: (07) 5519 8412. A referral can be made to ACAT by a relative/ representative over the telephone.
After a person has been referred to the ACAT, they will conduct an assessment and if appropriate, issue an approval to the applicant for either high or low care for either permanent or respite residency.
Application for Admission
There are three parts to the application:
- a) Application for Admission - completed by the person seeking accommodation or their representative.
- b) Medical Assessment - must be completed by a Doctor
- c) A copy of the ACAT Assessment must accompany the Application for Admission to enable the application to be processed.
Waiting List
Applications will be placed on our waiting list, and when a vacancy becomes available we will contact you. It is advisable to make regular contact with our staff to confirm that accommodation is still required. Urgent Admissions
We try to keep one or two places available for urgent admissions of persons with no ACAT approval, no referrals and prior paper work. This is our unfunded short term and emergency respite service. If you have an urgent need today please call us.
